If you have not attended SUNY Old Westbury for five (5) years or more, you must submit a new admissions application through either the SUNY Application or the Common Application instead of this form.

Please complete this re-admission application if:
✅ You were previously enrolled at Old Westbury but have not attended for two or more consecutive semesters.

Do not complete this form if:
❌ You have never enrolled in classes at Old Westbury.
❌ You are looking to roll over a past application from within the last year. Instead, email enroll@oldwestbury.edu to request a one-time rollover.

Key Notes for Re-Admittance:

Re-admission is only available for the Fall and Spring semesters. If you wish to enroll in Winter or Summer courses, submit this form for Fall or Spring and then email enroll@oldwestbury.edu to request Winter/Summer enrollment.
You must submit official transcripts from any colleges you attended after leaving Old Westbury.
If you were academically suspended or dismissed, you may be eligible for reinstatement after the required waiting period or by following the appeal process outlined in the College Catalog.
